We will contact you annually to provide feedback on how you and your assistance dog are doing as part of Can Do Canines’ ongoing effort to improve our assistance dog program. When you receive the annual survey via email, please complete it within the next two weeks.
At the same time, you will also receive a physical exam form (see link on the previous page) to bring to your veterinarian for completion at your dog's annual exam, and return that to us as well. The vet form does not need to be completed in two weeks. If your dog has been to the vet within the past six months, information from that visit can be used for the form.
Certified Team IDs will not be activated until both the Client Survey and the report from your veterinarian have been received and processed.
Can Do Canines is accredited by Assistance Dog International. This survey also helps us maintain our accreditation, as we are required to obtain annual feedback from clients after certification. This client survey and the information from your veterinarian are also required for you and your assistance dog to maintain your certification through Can Do Canines and receive your current ID.
As we continue to follow best practices as defined by Assistance Dogs International, we have moved to digital IDs for as many clients as possible. Digital IDs are more secure and convenient and will allow for greater flexibility in expiration dates moving forward.
