Raiser Feedback Form
Complete this form following a dog's stay with you for longer than 7 days and after a two-week prison break raiser assignment. Only use this form if you have taken the dog in public for training outings in a cape.
Get to Know This Dog
Complete this form when the canine in your care is going to a prison facility, moving into the FETCH program, or once they are called in for Final Training.
Behavior Evaluation Form
This form must be completed for all dogs in our program at 4 months and 15 months of age.
