Use these forms to make request with Puppy Program.

Host Request

If you need a host for the dog in your care, complete a Host Request.

Please submit your request at least two weeks prior to the date needed. We cannot guarantee that requests will be able to be fulfilled on short notice.

Supply Request

Complete the Supply Request Form if the dog in your care needs food, a distemper vaccination, or monthly preventative medications. Supply requests may also be filled at any in-person training opportunity. Supplies should be requested at least one business day in advance.

Pickup Location
Requested supplies will be available for pickup on the east side in the supply pickup shed ("Doggy Depot"). Feel free to park in the loading zone.

Request Public Access Training

Are you new to Can Do Canines Puppy Program or ready to take on a new role? Many of our dog hosting opportunities require volunteers to undergo Public Access Training to earn their “cape rights.” Cape Rights ensure volunteers are comfortable handling dogs in public places. Through this process, we will also provide details about the laws surrounding assistance dogs.

To request Public Access Training, volunteers must commit to certain roles, such as Raisers or Prison Break Raisers. If you are currently a Sitter who wishes to earn cape rights and take on new roles, please note that three Sitting assignments must be completed before receiving training.
